[Reverend?] and dear Sir:- The Bishop of Ohio directs me to write to you in regard to the Rev. [Reverend?] J.H. Cooperwaite, of The Andrew's Home, 346 Belmont Park, Montreal Canada. Will you be so good as to answer the enclosed questions, adding any other information that will help in forming a correct judgement and return to me at your Earliest convenience?
